NB 2.5 CLUTCH problems?

I was aware that the 2003 MX5 we bought recently was a model that was prone to clutch judder. Our one doesn't seem too bad here though and we are able to get nice smooth starts just by using a few more revs than normal. Just a few (well, 6 actually) questions about all this:

Is it normal to have to give a bit more throttle when starting off to match revs with clutch plate?
Did this clutch problem affect all 2003 NB 2.5 cars?
Was a Technical service Bulletin put out by Mazda NZ?
What was the cause, and was the fix effective?
What km's do most of you's get before clutch job?
